Nursing Diagnosis and Intervention for Hypertension Clients


Blood pressure is the force of blood pressing against the walls of your arteries. When it's too high, it raises the heart's workload and can cause serious damage to the arteries. Over time, uncontrolled high blood pressure increases the risk of heart disease, stroke, and kidney disease.

Nursing Diagnosis for Hypertension
Risk for Ineffective Tissue perfusion: Peripheral, Renal, Gastrointestinal, Cardiopulmonary related to impaired circulation

Nursing Action/Intervention  for Hypertension

  • Check Vital Sign Such As B.P(Blood Pressure)
  • Monitor Diet Plan less sodium low colestrol
  • Monitor input output Urin color
  • Give comfortable position
  • check Clients mental status (Stress can make your blood pressure spike)
  • encourage client's Avoid alcohol and smoking
  • Encourage relaxation techniques
  • To reduce daily activity
  • Maintain bed rest, elevate head of bed
  • Clients overweight(obesity) teach about diet and excercise
  • Give medication Diuretics,Beta-blockers,ACE Inhibitors,Calcium Channel Blockers and Other Medications as per doctor order
  • take deep breathing relaxation techniques
  • continue to monitor Clients blood pressure
